Do not invent grades, behavior records, or diagnoses. Student first name: [STUDENT] Teacher and class: [TEACHER] The one concern or bright spot the family can describe: [TOPIC] What the family already tried at home: [TRIED] Time they were offered: [TIME] Questions they do not want to ask in writing: [PRIVATE] Requirements: 1. Open with a two-sentence note that names [STUDENT], [TEACHER], and [TOPIC]. 2. Write 5 questions that fit a 10-minute talk. Each question should be answerable from the classroom. 3. Include one question about what is working, even when [TOPIC] is a worry. 4. Leave out [PRIVATE]. 5. If [TRIED] is present, add one question that builds on it. 6. If [TOPIC] names a label the family did not document, mark it [NEEDS INPUT] and do not expand it. 7. Close with thanks and the [TIME] they were offered. 8. Keep the whole message under 150 words. Deliverables: 1. Note the family can send 2. Five questions 3. A reminder of what to leave for the live conversation
